If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Scheduling
Our office hours are Monday through Thursday, 8:00 AM - 5:00 PM, and Friday's 8:00am - 2:00pm. We try our best to accommodate every patient’s needs. If you are in pain and need to be seen immediately, please tell our staff so your appointment can be scheduled as soon as possible.
When scheduling, please advise us of any medical condition and/or need for pre-medication. If you have dental insurance, please bring your insurance card to our office.
Patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Financial Arrangements and Dental Insurance
It is our desire to provide superior professional care and personalized service at the most reasonable cost to our patients. Because we are a one-appointment office, payment for care is required at the time treatment is rendered. For your convenience we offer a variety of payment options.

Dental Insurance is a contract between you, your employer and your insurance company. It is a means of sharing in the cost of your dental care. We will be happy to assist you in filing claims with the insurance company. Most insurance companies does not cover the total cost of your treatment so it will be necessary for you to pay your estimated portion of the fee on the day of the treatment. You may call our office and we will be glad to contact your insurance company to get an estimate of the amount they will cover.
